Confidentiality Terms and Agreement: All information shared during sessions is confidential and will not be disclosed to anyone without your written consent, except as required by law (such as threats of harm to self or others, or court orders). You also agree to respect and maintain the confidentiality of other participants in group or virtual sessions.
I consent to participate in virtual sessions and acknowledge that confidentiality may be limited in this format. I release The Distance, LLC and its staff from liability for technical disruptions, potential privacy breaches, or any outcomes resulting from participation.*
I acknowledge I have read, understand, and agree to the Disclaimer & Waiver and HIPAA Notice of Privacy Practices as posted on The Distance, LLC website.*
